In January 2009 we ran a workshop for managers concerned with implementing change following extreme, adverse, or ‘sentinel’ events such as accidents, misconduct, or system failures. The workshop was attended my senior managers from a range of sectors including: Health, Nuclear, Financial Services, Fire and Rescue, Air Transportation and Humanitarian Aid.
